CVS Caremark Charitable Trust Guidelines and Deadlines – 2012

General Guidelines:
Geography
The CVS Caremark Charitable Trust accepts applications from organizations where CVS Caremark has a business presence. This includes all states except: Colorado, Oregon, Washington, Idaho, Wyoming, South Dakota and Arkansas.
Grant Size
Grants amounts have ranged from $10,000 to $50,000 for one-year support.
IMPORTANT NOTE:
Grants can only be made to nonprofit organizations recognized by the IRS as public charities that are tax-exempt under Section 501(c)(3). CVS Caremark Charitable Trust is not able to provide grants to organizations that are classified by the IRS as a 509(a)(3) organization.
Use of Grant Funds
If a grant is awarded in the current calendar year (in this case, 2012), grant funds will be distributed for use in the following year (2013).

The CVS Caremark Charitable Trust will accept only one application per organization each year.
Grantee Expectations
Our goal is to create working relationships with our grantees. We view each grantee as a partner, working together toward a common goal. If awarded a grant, we will communicate with your team on a regular basis to make connections with our local teams, highlight your work on our website and leverage our relationship in a way that creates the greatest outcomes for your organization, as well as those that benefit from your work.
A year-end report is due from each grantee, which will focus on outcomes and successes. This report will be provided to grantees at the time a grant is awarded and is designed to track the successful implementation of the funded program. When possible we will identify grantees whose work is similar and bring them together to share information, best practices and key learnings during the funding year.
Additional Information
All applications will be reviewed upon receipt. The CVS Caremark Charitable Trust may request additional information from those organizations whose applications most closely align with our guidelines and the mission of the CVS Caremark Charitable Trust.
Only fully completed applications will be given consideration.
