CVS Caremark colleagues, who volunteer regularly at a nonprofit organization, may be eligible to apply. Challenge grants result in financial support for hundreds of charities each year. CVS Caremark Volunteer Challenge Grants are made possible by the CVS Caremark Charitable Trust, Inc.
We believe that CVS Caremark colleagues represent a great resource to their communities, and we are proud to support the organizations where colleagues volunteer. The goal of the CVS Caremark Volunteer Challenge Grant Program is to support our colleagues’ efforts in the communities where they live and work in the following areas:
• Pre-K-12 Education
• Health Care or Health-Related Efforts
• Community improvement (e.g., community clean-up, park renovations, or building playgrounds)
• Disaster Relief or Housing Construction for the Underserved
• General Community Support (e.g., food banks, soup kitchens, homeless shelters or agencies that provide social services to the underserved)
• Programs that Benefit Children with Disabilities
Who is eligible to apply?
• The applicant must be a permanent full-time or part-time colleague of CVS Caremark.
• Applications can only be submitted after 50 percent of the required volunteer hours have been completed.
• Before submitting an application for a Charitable Walk Grant, the walk event must be completed and
pledges received.
• The full 25 hours (for an individual application), 50 hours (for a team application) or charitable walk event
must be completed by November 30, 2010.
What size grants are being offered to the nonprofit organization?
• Charitable Walk Grant – $300
• Individual Grant – $500
• Team Grant – $1,500
What are the qualifications of a Walk, Individual or Team Grant?
• Charitable Walk Grant – Twelve or more CVS Caremark colleagues participate in a fundraising walk to support a nonprofit organization/school. Please note: each walk member must raise a minimum of $50 in pledges. Only one grant will be awarded for each walk event. CVS Caremark colleagues may apply for one charitable walk grant per calendar year. Walk event must take place from January through November 30, 2010.
• Individual Grant – A CVS Caremark colleague volunteers a minimum of 25 hours at one nonprofit organization/school from January through November 30, 2010.
• Team Grant – Five or more CVS Caremark colleagues volunteer a minimum of 50 hours at one nonprofit organization/school from January through November 30, 2010.
Can I apply for more than one grant from January through November 30, 2010?
No, each eligible CVS Caremark colleague may apply for one grant per year either as an Individual or as part of a Walk or Team application.
What type of organization is eligible to receive a grant?
• Public Schools (Pre-K-12)
• Nonprofit Organizations recognized by the IRS as public charities that are tax-exempt under Section 501(c)(3)
• Nonprofit Organizations/Schools providing services and/or programs in communities where CVS Caremark colleagues live and work
What type of organization is ineligible to receive a grant?
• Fraternal, Political and Membership Organizations
• Organizations that discriminate against a person or group on the basis of: age, race, ethnicity, sexual orientation, political affiliation, religious belief
• Easter Seals organizations and chapters are not eligible, because of CVS Caremark's national sponsorship of Easter Seals "Walk With Me" events
• Organizations providing services and/or programs in communities where CVS Caremark does not have a business presence
• Organizations that are classified by the IRS as a supporting organization 509(a)(3)
When will grant applications be accepted?
• Grant applications will be accepted starting January 1 through November 30, 2010.
How will grant application decisions be made?
Applications will be reviewed at the end of each month throughout the year. If all of the above-mentioned criteria have been met, the nonprofit organization/school will be awarded a grant. All colleagues will be notified of decisions in writing. When grants are awarded, checks will be sent directly to the nonprofit organization/school, not the colleague.
How can I apply for a CVS Caremark Volunteer Challenge Grant?
• Complete the two-part CVS Caremark Volunteer Challenge Grant Application.
Part One: Colleague Application Form – Select the correct Colleague Application Form that applies (Walk, Individual, or Team). This form must be completed by the CVS Caremark colleague submitting the application. In the case of a Walk or a Team Grant, one person should be designated as the coordinating applicant.
Part Two: Nonprofit Organization/School Application Form – This form must be completed by the nonprofit organization or school.
NOTE: Completed applications are accepted from January 1 through November 30, 2010, or until the annual budget is exhausted. Applications will be reviewed on a first-come, first-served basis. Grant decisions will be announced and grants will be awarded the month after a completed application has been submitted. Incomplete applications will not be considered – no exceptions.

The Volunteer Challenge Grant Application has been created as an interactive PDF file, which allows applicants to fill out the information in the PDF, and then print the completed application form locally. We are encouraging applicants to type in answers to the application vs. printing the application first and writing in the responses.
